MN School Immunization Requirements

The Minnesota School Immunization Law requires that each child in childcare, early childhood programs and school be protected from childhood diseases by receiving immunizations. All students in the Montevideo Public School District must have immunization records or an exemption record on file in the nurse’s office by the first day of the school year.

Immunizations include Diphtheria, Pertussis and Tetanus (DTaP/Td/Tdap); Polio; Measles, Mumps and Rubella (MMR); Hepatitis B; Meningococcal; and Varicella (Chicken Pox). Parents/guardians may file a medical exemption signed by a healthcare provider or a non-medical exemption signed by a parent/guardian and notarized.
